We are so excited that you are considering our office and team to provide you with individualized, functional, and integrated care to help you achieve your health and wellness goals whether they be acute illness resolution, management of chronic diseases, or to just ensure that you are living your life in optimal health.

If you are interested in becoming a new patient, please check with your insurance or view our accepted insurances here to see if we accept your insurance. If you do not have insurance or would rather become a Direct Primary Care member, click HERE. 

Once you have verified that we accept your insurance, please click the link below to fill out your initial intake forms. One of our team members will be in contact with you. Once your initial intake forms are approved you will be able to schedule a new patient appointment where further health information will be obtained prior to your visit to allow our healthcare team to prepare for your visit. 

Your first appointment as a new patient will be an office visit. It will be subject to a co-pay, co-insurance or deductible, depending on your insurance plan. First appointment availability varies, depending on which provider is requested. You will receive an email reminder for your first appointment, through which you will complete the remainder of your patient registration and health history through online check-in.  If you don’t have access to a computer or the internet, you can complete this process in the office prior to your appointment.  Please arrive 30 minutes before your appointment time to allow plenty of time to complete the online check-in. Failure to complete your forms prior to your appointment or failure to arrive early enough to complete your forms before your scheduled time with one of our providers may mean that you will need to be rescheduled for a future appointment date and/or time.
