Patient Policies

Appointment Policy

New Patients: You must bring a valid form of government-issued photo identification such as a Drivers's License, State ID, or US Passport, which must include your full name. Your id cannot be expired. The name on the id must match the name on the insurance card. Also, be sure to bring all insurance cards and previous medical records needed for your appointment. Please plan to arrive at your appointment 20 minutes early to complete the necessary paperwork and enjoy the amenities. If you are more than 10 minutes late for your appointment, you will need to be rescheduled. This is to ensure that our providers have the necessary time to provide you with exceptional care and respect the time of other patients. If you need to reschedule or cancel your appointment for any reason, please give a minimum of 24 hours' notice. If you are unable to make more than two initial appointments, please contact our CEO for further scheduling.

This is to ensure that our providers have the necessary time to provide you with exceptional care and respect the time of other patients. If you need to reschedule or cancel your appointment for any reason, please give a minimum of 24 hours' notice. If you are unable to attend your appointment without providing 24 hours notice or are more than 10 minutes late to your appointment, a $30 fee will be applied to your account. This fee is not covered by insurance, and it is your responsibility. 

Financial Policy

Fees for services are due before being seen by one of our providers. If we accept your insurance, we will make every effort to collect payment from your insurance company; however, you are still responsible for any fees not reimbursed by your insurance provider. All balances are due before scheduling an appointment. If you need assistance with your bill, or you have additional questions, please contact our CEO.
